Parent/Carer Declaration
1. Free School Meals are provided at West Berkshire schools, nurseries and Pupil Referral Units (PRUs) to children whose parents/guardians are in receipt of
  1. - Universal Credit (provided you have an annual net earned income of no more than £7400, as assessed by earnings from up to 3 of your most recent assessment periods)
  2. - Income Support
  3. - Income Based Job Seekers Allowance
  4. - Income related Employment Support Allowance support under part VI of the Immigration and Asylum Act 1999 (from the National Asylum Support Service)
  5. - the guarantee element of State Pension
  6. - Credit Child Tax Credit, provided you're not also entitled to Working Tax Credit and have an annual income of no more than £16,190
  7. - Working tax credit run on-paid for four weeks after you stop qualifying for Working Tax Credit
3. This form uses an automatic central Government Eligibility Checking Service (ECS) to check your eligibility for free school meals only. The use of this service is permitted by sections 110 and 111 of the Education Act 2005 and it is an offence to use the service for any purpose other than checking eligibility for free school meals.
